  • This online utility pre-design review form must be submitted with all Type D7 or D8 permit applications for underground utility work if the answer is "yes" to any one of the following three questions. Does the proposed underground utility work include:

    (1) cutting the existing paved area of any ARTERIAL road?
    (2) Propose to install, repair, or replace more than 2,000 LF of underground utilities within a county NON-ARTERIAL roadway?
    (3) abandoning a utility facility in the public right-of-way?

    • How do I fill out this online utility pre-design review form?

      Answer the questions on each page of this form as completely and accurately as possible and then click on the Next button located at the bottom of each page to continue to the next page. You will not be allowed to proceed to the next page unless all of the required form fields are filled in. Form fields that are required to be filled in have a red asterik * next to them. When you reach the final page of this online form, the Next button will not appear. The final page includes instructions on how to submit this online form for review.

      Can I save an incomplete form to fill out and submit later?

      Incomplete forms with missing data in required fields can't be submitted. Incomplete forms can be saved and returned to at a later date. To save a form click on the Save button that is located at the bottom of any of the pages and instructions will be provided.

      How long will it take to complete the form?

      It depends, but most applicants are able to fill out the form and submit it in 15 minutes or less. The form contains up to 45 questions but not every applicant will be asked all 45 questions. The form is set up to ask for only the information that is required to complete the review and some questions or information fields only appear based on information in form fields or answers to questions provided by the applicant on previous pages of the form.

      For example, there are more than 15 questions that appear for an applicant must answer if an applicant selects the "yes" button next to Screening Question #3 which asks if the applicant is proposing to abandon a utility facility in the public right-of-way. If the applicant answers "no" to Screening Question #3 then the 15 related question and information form fields will not appear o

  • UTILITY LOCATES AND NOTIFICATION REQUIREMENTS
          1. Call 811 to locate signal loops, and
          2. Notify the DPW Signal Shop at 425-388-7551

  • SIGNING AND STRIPING RESTORATION REQUIREMENTS
         1. A channelization and/or sign plan must be provided in the project plans.
         2. The Applicant must restore the signing and striping per the approved plan.

  • New "Hard surface" means  new areas of impervious surface and permeable pavement. Common impervious surfaces include concrete or asphalt paving, graveled areas and roads, packed earthen materials. See SCC 30.91H.035. New Hard Surface does not mean restoration of existing pavement.

  • DRAINAGE CODE COMPLIANCE
    The proposal must be reviewed per SCC 30.63A for drainage code compliance.

  • TEMPORARY TRAFFIC CONTROL REQUIREMENTS
         1. A Lane Closure (Type B5) permit is required. See Bulletin #102 for more info.

  • Justification for abandonment of utility facilities

  • Snohomish County Code (SCC) 13.01.040(3) prohibits the abandonment or discontinuation of use of any structure, object or feature within the right-of-way without the prior written approval of the public works department. Each utility franchise agreement between the County and an individual utility may have specific requirements related to abandonment.

    The County Engineer will review proposals to abandon utility facilities within its right-of-way on a case-by-case basis given the requirements in place related to the County Code and franchise agreements to determine if there is sufficient justification for utility abandonment. The answers provided with this questionnaire will be considered when determining if there is sufficient justification to approve abandonment of utility facilities in the right-of-way.
